The human factor in the decision making process is the main cause of incidents, even involuntary, directly connected with the use of the product itself. If the accident rate is to be decreased, human factor must be better understood and the knowledge more broadly applied. Purpose of this article is to present an overview and summary of experiences acquired over the years in the different phases of the lifecycle of projects and incidents analysis. ER -
